This Consumer Request Form is intended for individuals who are making a request pursuant to the California Consumer Privacy Act (CCPA). The information provided in this form will be used to verify your identity through an UNCLE representative and to fulfill your request. If the information provided in this form is insufficient to verify your identity or accurately respond to your request, UNCLE CU may contact you to request additional clarification and detail.

Please Note:

• If you are submitting this as an Authorized Agent or a Minor (under 13 or at least 13 years of age and less than 16 years of age) a completed consent form must be submitted along with this CCPA request form in order for this request to be processed. Please visit our California Consumer Privacy Page to find the applicable consent forms.

• If you have a valid Power of Attorney no additional consent form is needed. Please attach the Power of Attorney documentation to this form.

• Any completed consent form or supplemental documentation should be attached at the bottom of this CCPA form (attachment box).

Our goal is to respond to your request within 45 days from receipt. The timeframe for response may be extended by an additional 45 days, if reasonably necessary, to fully and accurately respond to your request.
